Quartus II Simulator is not supported from Ver.10.0 onwards.
If you really want to use the Quartus II Simulator, you need to update the Quartus II version to Ver.9.1sp2 or earlier.

ModelSim-Altera Edition (paid) or ModelSim-Altera Starter Edition (free) will be used for Ver.10.0 or later.
This is not a waveform input, but describes the input conditions in RTL (Verilog or VHDL).
