The Intel HEX data generated by the general-purpose tool may not be read correctly.

Check if the following settings have been made. :
1) Launch Quartus II and select Options from the Tools menu
2) Highlight Memory Editor in the Options screen
3) Check Read or write Hexadecimal Files [.hex] Using byte addressing [Intel format].
