If you are using the Embedded Reset Controller located in the Custom PHY, the possible cause is that Reset Per Channel is not enabled.
The Custom PHY GUI does not have an item to enable the above, so please connect the Reset Controller with Reset Per Channel enabled to the Custom PHY.
