In ModelSim-Altera, I created a stimulus (testbench) with the Wave editor, but the waveform is not displayed correctly on the editor.

ModelSim-Altera Edition and ModelSim-Altera Starter Edition 10.3x (such as 10.3c and 10.3d) have a glitch when creating stimulus with the Wave editor.
The waveform information set in the Create Pattern Wizard (as well as the wave create command) is not displayed correctly on the Wave editor.
Even in that state, you can generate a testbench file by File menu ⇒ Export ⇒ Waveform execution, but it cannot be guaranteed that the set contents are correctly reflected in the description.
Therefore, please write the testbench by yourself.
Alternatively, use a different version of ModelSim-Altera for testbench creation only, and use the latest (or desired) version for compilation, simulation execution, etc.

If you create a testbench using the Wave editor flow and use it for simulation verification in 10.3x, check the description of the generated testbench file and the waveform displayed in the Wave window by running the simulation. Be sure to check if the test pattern is what you want.
ModelSim-Altera Edition and ModelSim-Altera Starter Edition 10.3x are versions that support Quartus® II v14.1 and v15.0.
